Blog Summary

Only 20% of medical group leaders say their organization has formal AI governance, and Nikki Ballinger sees that gap matter most on the patient access line, where a voice AI agent verifies callers, writes appointments into the EHR, and decides when a caller needs a person. Drawing on Parlance implementations at hundreds of health systems, Nikki lays out three decisions to make before scaling an agent to every patient call. First, scope the agent like a new hire, with its own identity, access only to what its job requires, and a written list of what it does not do. Second, treat disclosure as a design requirement, with a short, plain introduction and an immediate path to a person. Third, build a call-level audit trail that shows what the caller asked, what the agent changed, and why it did or did not escalate, because without that record a containment rate is a number no one can explain.

Your Voice AI Agent Can Book Appointments. Who Decided What Else It Can Do?

Agentic AI is reaching the patient access line faster than governance can follow. Scoping, disclosure, and auditability determine whether it scales safely.

AI has moved into everyday healthcare operations faster than most organizations have written rules for it. In a January 2026 MGMA Stat poll, only 20% of medical group leaders said their organization has AI governance or a formal AI policy in place. That gap matters most on the patient access line, where an AI agent talks with patients in real time.

A voice agent verifies a caller’s identity, writes appointments into the EHR, and decides when a caller needs a person. Each of those actions is a permission that someone granted, either deliberately or by default. Our earlier post on voice AI governance after go-live covered the operating model for production. This post covers the three decisions a health system should make before expanding a voice AI agent from a pilot to every patient call.

Scope the Agent the Way You Would Scope a New Hire

Parlance has implemented voice AI at hundreds of health systems, and in my experience, the best rule is simple: a voice AI agent should be able to see and change only the information it needs to do its job. A scheduling agent that books and reschedules visits needs access to provider availability and specific appointment types. That scheduling agent does not need visibility into test results or clinical notes.

The agent also needs its own identity. A voice agent that borrows a staff member’s login or a shared account puts patient privacy at risk, because no one can tell the agent’s actions apart from a person’s. A dedicated, non-human identity with its own role in the EHR is far easier to monitor, restrict, and retire. Parlance AI agents connect directly to EHR and scheduling systems, and each connection should be scoped to the workflow the agent supports.

What this means for health systems: write the agent’s job description before configuring the agent, and document what the agent does not do with the same precision as what it does. The second list is where most governance failures begin.

Treat Disclosure as a Design Requirement, Not a Legal Footnote

States are starting to require AI disclosure in healthcare. Texas, for example, now requires health care providers to tell patients in plain language when AI is used in their care under the Texas Responsible Artificial Intelligence Governance Act. Health systems that operate in several states should build one disclosure standard that meets the strictest rules they face. Counsel decides which rules apply, and the design team decides how the disclosure sounds.

Patients expect to know when they are speaking with AI, even on a simple scheduling call, and a short, honest introduction builds trust from the first seconds of the call. Callers with hearing, cognitive, or language differences are the least able to absorb a long legal statement, so effective disclosure is brief, plain, and paired with an immediate path to a person: “You’re speaking with the automated scheduling assistant for [Health System Name]. I can help you book or change an appointment, or connect you with our team at any time.”

Build a Call-Level Audit Trail Your Patient Access Team Can Review

A standard IT security log only shows that the voice AI agent connected to a system, such as the EHR or the scheduling platform, and when it connected. Patient access leaders need a call-level record that shows what the caller asked, what the agent understood, which patient record the agent touched, what the agent changed, and why the agent did or did not escalate. Without that record, a containment rate is a number no one can explain.

That record depends on three disciplines. First, log every action the agent takes in the EHR, along with the caller’s original words. Second, define escalation rules explicitly, such as failed verification or repeated misrecognition, and review a sample of calls on a fixed schedule. In Parlance deployments, two failed attempts trigger an automatic transfer to a live agent. Third, monitor the phone lines themselves, because an agent that silently stops answering is also a governance failure. As Katie Cardarelli explains in her blog on moving voice AI from pilot to production, that review only works when a named team owns it after launch.

Three Questions Before You Scale Voice AI

The health systems that scale voice AI safely will answer three questions first: What is this agent allowed to do? How will patients know they are speaking with an agent? How will we see everything the agent did? At Parlance, we have spent more than 30 years building voice for the front door of health systems, and the principle has not changed. Every caller deserves a system that identifies itself clearly, accesses only what the task requires, and accounts for every action it takes.
