The Digital Front Door is Locked for Millions of Patients. The Phone is Still the Key.

Health systems spent a decade building a beautiful entrance. But millions of patients still can’t get through it.

If you are a health system executive or your organization supports the healthcare industry with technology or services, you are familiar with the phrase: digital front door. Portals, apps, and chatbots…these are tools that promise to ease the patient journey and optimize business operations. The category is real, and it’s growing… Analysts size the digital front door market at about $31.66 billion in 2026, on its way to more than $80 billion by 2031.

The truth: millions of patients have a key they can’t use because the new healthcare front door still has an old, rusted lock and it’s quite challenging to turn the key.

Not metaphorically. Literally. While health systems have spent billions on new portals, most have not modernized the phone, and it’s the channel used by most of the U.S. population. Hospitals and clinics are not making it easy enough for people of all ages and abilities to make appointments, get the information they need, solve complicated problems, and access needed support. Not the regional health systems, nor the national IDN brands.

Does your access strategy assume every older patient has the digital literacy to navigate a portal, the vision to read a mobile app, and the English fluency necessary to complete every step in their transaction? Does your access strategy assume that every person managing care for their kids, themselves, and maybe an elderly parent too, has the time to wait on hold? If so, you’ve built an entrance that a large share of your patient population cannot walk through without friction and frustration.

The People Locked Out Aren’t Edge Cases, It’s a Big Slice of the U.S. Population

They’re your highest-need patients. Older adults managing the symptoms of multiple chronic conditions. Medicaid populations. People with disabilities. Patients who speak a language your new app doesn’t. As of May 11, 2026, updated ADA Title II rules require digital services from public entities to meet real accessibility standards — a clear signal that “download our app” is no longer a defensible access strategy by itself.

The Center for Medicare & Medicaid Services is moving in the same direction. The ACCESS Model launched in 2026 to expand technology-enabled care for traditional Medicare patients with chronic conditions, and the Medicare App Library opened earlier this year to help older adults find tools they can use. The regulatory message is clear: access has to reach everyone, not just people who are digitally fluent. The phone MUST be modernized as a part of the digital front door. Given the majority of American consumers engage with their doctors and healthcare needs by calling on the phone, optimizing this channel has widespread impact on patient experience.

Who gets left behind when digital literacy is required? A meaningful slice of the population is the answer to that question!

The other segment of people who can get locked out are working parents. People who have no choice but to attend to their own healthcare needs, while also managing healthcare for their children (and maybe even aging parents) — all in the cracks of time between 9AM and 5PM. Many of us wish we could stop the clock to attend to personal business. None of us want to be frustrated waiting on hold, and none of us want to wait to talk to a person if we need to ask a simple question or quickly make an appointment.

The voice is the most natural user interface. When voice AI is deployed on the phone, almost anyone can speak naturally, ask for what they need, and easily accomplish routine tasks like making an appointment or getting an answer to a frequently asked question. When voice AI makes self-service on the phone frictionless, the percentage of people using it increases substantially, and those patients who require complex support or empathy can get to a human support person without waiting on hold.

Healthcare Stopped Investing in the Phone

While budgets flow to apps, the phone quietly remains the front door most patients use. And in too many health systems, it’s the most neglected channel in the building.

You know the experience, because you’ve lived it. A menu tree from 2009. “Please listen carefully, as our options have changed…” A hold queue that outlasts everyone’s patience. According to HFMA (Healthcare Financial Management Association) benchmarks for healthcare call centers, average hold time runs around 50 seconds, and abandonment rates as high as 15% are considered within the acceptable range, meaning as many as one in seven patients who call simply hang up. A significant share of appointment requests arrive outside business hours, when a legacy phone system offers nothing but a voicemail box and a callback that may come days later — or weeks later.

Legacy IVR was never built to be a front door. It was built to route calls and contain cost. Treating it as an actual access strategy is like putting your grand entrance behind a 1980’s turnstile that only takes exact change. Technically it’s an entrance, but practically it’s a real barrier.

A Front Door Has to Open for Everyone, or Too Many Patients Experience a Barrier to Entry

This is where modern voice AI demonstrates its worth — not as another shiny channel, but as the one that finally makes the most universal channel work. A patient doesn’t need an app, a login, or a tutorial to speak in their own voice. Conversational AI on the voice channel can schedule, route, and resolve — in natural language, around the clock, in the patient’s own words. Callers are understood, answered, and helped. And when a human agent is needed, they are readily available given the major reduction in call volume.
